- Treatment: to provide and coordinate your dental care, including with specialists and dental laboratories.
- Payment: to bill you, your insurance plan or a third party.
- Health care operations: quality review, training and running the practice.
- As required by law, for public health activities and in other situations permitted by HIPAA.
Other uses, including marketing, require your written authorization, which you may revoke at any time.
Your rights
- Get a copy of your dental record and request corrections.
- Request confidential communications and limits on what we share.
- Get a list of certain disclosures we have made.
- Get a paper copy of this notice and file a complaint with us or the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
